Troop Patrol Form : Used by scouts for creating a list of scouts in each patrol. Patrol Camping: This form is used for trip planning : Includes: Duty Roster, Equipment List, Menu Planner and Shopping list. Media Release : This form is signed by scout parents giving us permission to use the scouts image in photo, video and audio presentations. Personal Data: This form is completed by parents. Includes Name, contact information and driving information. MB Counselor App.: This form is used to apply as a Merit Badge counselor. BSA Medical Form: This form is the official medical form need to attend Yawgoog Summer Camp.
